Welcome to Our Online Parent Referral System for Health Care Providers!
Our online referral form can be used to let us know about parents who are expecting a baby with Down syndrome or parents who have a new baby with Down syndrome. If you are making a referral for the parents, please make sure you have obtained their permission first.
Parents referred to us can receive any of the following services through our Parent Support Program:
- Books and information about Down syndrome and resources for expectant or new parents
- Connection to a trained parent mentor
- Information about programs and resources in the Chicago area
- Referrals to other Down syndrome groups and organizations
